DOT PHYSICALS

Department of Transportation Physicals completed by a certified medical examiner on the National Registry.

DOT Physical

$80.00

DOT Physical - please bring driver's license, all medications, A1c, CPAP Compliance letters, special tests, clearances, glasses, contacts, and hearing aids.  

3 Month Card Re-exam

$40.00

This is for a physical being re-done that was issued for an initial 3 months. (1st time) The initial exam is still $80.00 even if only 3 months is given. 

DRUG AND ALCOHOL TESTS

We provide drug and alcohol testing for DOT and NON-DOT for businesses. You must have a CCF form or FormFox Authorization form for us to use. We are able to store CCF's and Supplies at out location for your convenience!

DOT URINE, Non-DOT Urine Collection ONLY

$25.00

This Drug testing option is just for collection services only!  This means you have your own lab and MRO and we are just collecting a sample and sending it to the lab. 

Breath Alcohol Screening

$30.00

DOT or NON-DOT Breath Alcohol Screening - Performed using an approved Breath Alcohol Testing Device by a certified BAT. 

Breath Alcohol Confirmation Test

$30.00

DOT or NON-DOT Confirmation Testing if a Screening was positive.
